Best thing you've done here: Saved a Korean national's life last year. I received a 911 call regarding someone who had difficulty breathing. Upon arrival on scene, the patient presented with symptoms of a heart attack. We were able to transport the patient to the closest off-base hospital and he survived.

Reason for nomination: "Sergeant Reyes was the PHA point of contact for 40 Wolf Pack First Term Airman's Center students, who had their health record review updated in two hours. He was notified of only 30 individuals, but 40 showed up and he was still able to accomplish the eye, lab and blood pressure requirements for all. Not only did he nail this process, in doing so he enabled them to make their next scheduled appointment on time. Also, Sergeant Reyes reviewed 15 medical records and developed electronic Form 2766's with only four hours' notice in direct support of Operation Max Thunder, on the same day along with the FTAC class tasking. To further highlight his skills, Sergeant Reyes still managed to in-process 10 new 8th Fighter Wing personnel and complete an additional 23 PHAs, bringing the wing's HRR rates up from 98 percent to 99 percent.
